Do some research on DC-DC converters. They are very complicated but, have a very small parts count excluding the oscillator. All it need is an inductor, diode, capacitor, switching transistor(i usually use a 2n2222 for low v) and an oscillator.

It may be just a CW cascade, or a little inverter driving a cascade (which can use considerably smaller caps). Or it is just a little driver and a small HV transformer (such as those used in photocopiers).

I took a thing very simmilar to that out from an air freshner. I was small, (2x2x4 cm).
OT:
When connected to an 9v battery, it makes a nice "pikashoe", I connected the negative terminal to me, and the positive to alluminium foil under my shoes. Then if it is on I will collect a static charge and can shock people who are sitting or not using shoes. (Or everyone at a good day, with low humidity)
Not like I go around shoking people, but the device is fun. I have a switch inside my shoe so I can turn it on and off without anyone noticing. It can also be used to "bend water" and levitate small pices of paper.

Interesting. I guess that only works with plastic soles. But doesn't it look strange when a wire is hanging out of your shoe?

Lol, OT again, but they use the same thing in rockband drums. You do not want a switch because they have too much mass. So, piezoeletric pads is what they use, because it takes very little voltage to trigger the game, therefor there is little movement required.
Sorry if that was unclear, i typed half then stopped then started again :P.

Hehe, It is a normal leather shoe, (with rubber soles). All the wireing is concealed within the shoe/stipped to my legg under my jeans The only drawback is the swich under my toe is making walking slightly uncomforable.
