To proceed with a C&W at ELF can only mean one thing - the use of capacitors of enormous size.
A picture tells a thousand words, so here is Henning Ulmland's 160kV C&W X-ray PSU working at the 50Hz mains frequency - just imagine how much each one of those giant capacitors must have cost him!
Another constructor using 50kHz instead of 50Hz might have been able to get a 160kV 5mA supply into the volume of just one or two of Henning's jumbo capacitors.
