Well if you can post the design we can take a look at it and let you know.. Also as for arcing i've had it happen from not having smooth cuts. You can get break out points on the bridge if you have any sharp points. Also watch out for stray inductance on your bridge as it can greatly increase the voltage there.

Also be extremely carefull with the flux - either use conventional solid flux or, in case you are using grease fluxes, carefully clean up the PCB with alcohol, preferably in an ultrasound bath. Wipe the board with alcohol afterwards. I had an alike issue with my voltage sence circuit which was mounted on a protoboard (hehe yeah, a real spagetti :D) - the flux caused some minor currents across the LED control MOSFET pins and the cleanup procedure mentioned above (had no US bath btw so just held it in alco for a while) helped perfectly.
