I'm trying to plug my NST into this old AC voltage regulator that I found but it doesnt have a hole for the ground. Does anybody have any suggestions for fixing this problem?

See if there is any connection from the input plug to the case. If there isn't, you can out a ground wire to the case and use the case as a ground connection. I think it would be a good idea to do for safety reasons.

You can just connect the nst ground to a water pipe. The ground has no effect on the variac but you might want to ground that to for safety reasons. Also i would suggest you resize those pics before a mod bites you.

If I use a 2 prong adapter to plug the NST into the variac and ground the NST to the case, and then use a 3 prong adapter to run a ground from the case to the wall plug, is there going to be a risk of shock when I use the variac?
(does this even make sense? sorry, this isn't my area of expertise. I'm learning as I go)

This is suffecient grounding to avoid being killed, you will not avoid getting shocked as it will be you that makes your current leakage breaker trip :)
