A funny thing happened this morning at work; a colleague was working on a h.f. welder (3-phase input, bridge rectifier, 3x large electrolytics, igbt inverter) when a LARGE bang echoed around the factory. I turned around to look ... smoke from the igbt.
Another colleague came over and asked 'what happened ?'
" I was measuring the voltage when I slipped and the probe shorted the bus like this"
..BANG !
It was only funny because no one was hurt, and I was watching as he demonstrated.

Hehehe! I heard a similar story about someone who wired up a power supply wrong. capacitor exited at high speed, through the wall of the cubicle and went between the newspaper and eyes of the French technician in it, with "MERDE!!!!" exclaimed.

The other day some one at work was pushing a 600v breaker cubical into live switch gear and it bottomed out. He's lucky the frame has about 1200mm of metal in contact with the grounded frame or he would have been cooked. 600v is great for high power equipment but switching is a nightmare.
