Look at your layout. Specially look at how the ground connections were made. You have a high-voltage section controlled by a low-voltage circuit. Any interference due to proximity, ground currents, etc. can lead to unexpected feedback and oscillation. You don't need the operational amplifier to make the circuit work. Try to connect Vc, with a few volts more, directly to the gate. One less problematic feedback.

It is absolutely true that the power supply is directly referenced to bldg/earth ground and that the current regulator is referenced to the case ground, then to bldg/earth ground. (Also the switched 24V and 15V supplies have their common/return tied to case ground). I thought this was pretty much necessary for safety and for this type of regulator. While I have considered this possibility as the source of the problem, via ground noise, I'm not sure of a way of testing whether this is the primary culprit.

EDIT: I'm considering installing an isolated ground receptacle in my lab; however, I'm not sure if the power supply (high voltage) or the current regulator (low voltage) should utilize it to reduce ground intereference problems. For safety, I think that the HV supply should use this receptacle. Sound right?
