i dont think this is so high, your board layout may be good enough.

what about current how much is being switched? stray inductance can cause spurious voltages to appear across traces.

At the duty that is shown (the screen is significantly zoomed in), there is less than 0.5A.

I am wondering if the ceramic 0.1uf bypass capacitor on the FOD3180 is adequate. How can I tell if the FOD3180 is the problem? Could there not be enough gate current available? Why does the problem resolve when the duty increases?

I'm going to reevaluate the input to the FOD3180 and see if the problem is on the input end of the chip as I've been focusing on the output side.

Gate drive ICs should be bypassed with a low ESR electrolytic right next to the IC, a ceramic isn't high enough capacitance. I used 330uF per gate drive chip in my DRSSTCs.

first put 100-500uF electrlytic in parallel with as large a film type as you have or can fit, lets see if that solves it, steve mcconner is right the gate drive ic suck large charge in shrot time periods so you need to have a cap close .

Electrolytic is best for the job, they have high capacitance and slightly higher ESR than the other types, so any ringing gets damped.

I think that a 10uF tantalum + 100nF ceramic right next to the IC supply pin is the best choice...
